        1. 6 copies of "Each" CD you wish to list with MainStreet. (One copy is for our use,
            the other 5 copies are for our warehouse and distribution stock for your sales.)

        2. Two of Your Artist Photographs for Your MainStreet Pages.

        3. There is a $150.00 annual fee for each CD you wish to list on Mainstreet.

            For listing "Each CD" we accept: Check, Money Order, or Your Visa or
            Mastercard. The Cost is $150 annual fee for "Each CD" you want to
            register with MainStreet CD. (Please, Include expiration date, Month and Year, if using 
            your credit card.)

            We sell CD's for $15.00. We keep $7.50, and send you $7.50 for each CD Sold through Us.

            All Accounts are paid twice annually ( January 1, July 1, )
